Changes in season🍂🍁 usually bring about some change in life... I hope that this is positive change for you.

For PA patients, we ask that you email the PA provider (QCMD@drqcampbell.com) to make an appointment online until further notice.

NJ patients:

Online booking will be available on Tuesday, Wednesday, and Thursday. If you require booking at different times, kindly contact the NJ provider via email.

Dr. Chandler vacation times: 10-15 to 10-21, 10-31 to 11-11-24.

We will also be away Thanksgiving, Christmas, and New Year's holidays. We will update you for March and beyond, with another blog post....

NJ:

Need to renew your card? Go to njmmp.nj.gov.

You will need your reference number (alphanumeric). Keep those initial emails! If you need this number but cannot find it, email us. You also created a username and password on the CRC website. We do not have access to this information, this is only through the CRC. Remember, whether you have a physical or an electronic card, it lasts for two years. However, the state recommends renewal of monthly allotments every 360 days from your provider.

Once you make an appointment, you will receive an email (check your junk/spam) with instructions on how to pay, fill out online forms, and complete the video visit. Make sure you complete online forms at least 15 minutes prior to your appointment. Remember returning patients only need to fill out required sections or new changes.

Financial Trouble?

NJ CRC has in-person clinics (Trenton, Newark, AC) for those who cannot afford their doctor appointments.
