COMPLETE VISIT ONE
Your online paperwork is completed. Your fee is paid. You have your link. You will complete the appointment with the doctor. They will go over any drug interactions or concerns. Then schedule your Education visit.
Congrats you have created your first visit and now you are ready for your educational and administrative visit. This is in a group visit or webinar format. After this you will receive a registration code in an email within in 24 hours to register with state.
You will learn about cannabis as a medicine. You will be ready to register online. You will need:
Photograph (Passport- Style)
Taken within the last 60 days; Taken against a plain, white background such as a wall; No hats/glasses; Full-face view; Do not use any photo filters
Government Issued photo ID (Choose one of the following):
Current NJ Driver's License/Non-Drivers ID issued by MVC; NJ County ID showing patient name and address
Proof of New Jersey Residency (Choose one of the following):
Utility Bill issued within the 90 days; Correspondence from the Internal Revenue Service (IRS)/NJ Division of Taxation, issued within the year; Federal, State, or local government correspondence issued to the applicant within the 90 days; Bank statements issued within each of the three months; Proof of government assistance (if applicable)
Your MMP card fee is paid to the NJ Department of Health. The fee is $20 or $100 based on age, vet, disability, or financial status.

FOLLOW UP ASSESSMENT
This fulfills our four visit state-mandated requirement and also checks for any side effects, adverse reactions or issues. Your follow up assessment should be completed no longer than 6 months from your first visit. Your doctor will determine, based on your risk, if your follow up will be in 1- 6 months. After this, your follow up schedule will be yearly, unless the law changes or you are high risk. Check out our FAQs for more info.
