DO I HAVE A QUALIFYING CONDITION?
ALS, Anxiety, Cancer, Chronic Pain, Dysmenorrhea (Painful Cramps), Glaucoma, Inflammatory bowel disease, including Crohn’s disease, Intractable skeletal spasticity, Migraine, Multiple sclerosis, Muscular dystrophy, Opioid Use Disorder, Positive status for Human Immunodeficiency Virus (HIV) and Acquired Deficiency Syndrome (AIDS), Post-Traumatic Stress Disorder (PTSD), Seizure disorder, terminal illness with prognosis of less than 12 months to live, Tourette Syndrome are eligible for medicinal cannabis in NJ.
DO I HAVE PROOF OF THE CONDITION?
A prescription, patient portal screen shot, or any documentation from a medical professional or insurance company will suffice.
BOOK AN APPOINTMENT ONLINE
If virtual, new patients will book an online seminar. A 45 minute education session to begin your journey with medical cannabis. You will also complete online forms and a one-on-one visit with a provider to review your medical history. If you have an in-person in our Lawnside office, this will be an one-on-one, you will have handouts and a recording to take home to review with you as well.
MOST OF OUR VISITS ARE VIRTUAL BUT THERE ARE OPTIONS.
I love to administer education virtually. You will receive a link to a virtual telemedicine visit and a welcome email.
Get ready. Google Meet will work from a PC or laptop. If you have a cell phone or tablet, you will need to download the app.
Our forms are virtual as well. Please complete the online paperwork before your visit.
If you are unable to complete visits virtually, we have monthly in-office visits, when feasible, are at location in Lawnside, NJ: 106 Warwick Rd, Lawnside, NJ 08045. Please note there are 3 steps to enter.
Your online paperwork is completed. Your fee is paid. You have your link. Your education session is complete. The provider will go over any drug interactions or concerns during your appointment. We recommend that moderate and high risk patients have a follow up visit, but this is not required.
DO YOU QUALIFY FOR A DISCOUNT?
The state of NJ gives a discount for those on disability, over 65, receive SNAP or Medicaid benefits. We do as well, email the medical director for more information on our current discounts.
GO TO THE DISPENSARY
The state reviewed and approved your application and you have a card - you made it! Check their website before going. Also we recommend calling for an orientation to the dispensary's process. Make sure you have all their resources too! Informed patients make Smart Cannabis patients.
