DO I HAVE A QUALIFYING CONDITION?
ALS, Anxiety, Cancer, Chronic Pain, Dysmenorrhea (Painful Cramps), Glaucoma, Inflammatory bowel disease, including Crohn’s disease, Intractable skeletal spasticity, Migraine, Multiple sclerosis, Muscular dystrophy, Opioid Use Disorder, Positive status for Human Immunodeficiency Virus (HIV) and Acquired Deficiency Syndrome (AIDS), Post-Traumatic Stress Disorder (PTSD), Seizure disorder, including epilepsy, Terminal illness with prognosis of less than 12 months to live, Tourette Syndrome
DO I HAVE PROOF OF THE CONDITION?
A prescription, patient portal screen shot, or any documentation from a medical professional or insurance company will suffice.
BOOK AN ONLINE APPOINTMENT
New Patients will book an online seminar. A 45 minute education session to begin your journey with medical cannabis. After, you will complete a brief one-on-one visit with a provider to review your medical history. At this visit, you will be provided with all necessary requirements to complete your registration.
ALL VISITS ARE VIRTUAL.
You will receive a link to a virtual telemedicine visit and a welcome email.
Get ready. Google Meet will work from a PC or laptop. If you have a cell phone or tablet, you will need to download the app.
New Patient Paperwork is now virtual as well. Please complete the online paperwork as completely as possible before your visit.
Your online paperwork is completed. Your fee is paid. You have your link. Your education session is complete. The provider will go over any drug interactions or concerns during your appointment. We recommend that moderate and high risk patients have a follow up visit, but this is not required.
DO YOU QUALIFY FOR A DISCOUNT?
The state of NJ gives a discount for those on disability, over 65, receive SNAP or Medicaid benefits. We do as well "2021MMP." We will require proof or you will charged the remainder of the discount.
