DO I HAVE A QUALIFYING CONDITION?

ALS, Anxiety, Autism, Cancer, Chronic Pain, Glaucoma, Hepatitis C (chronic), Inflammatory bowel disease, including Crohn’s disease, Intractable spasticity, Migraine, Multiple sclerosis, Muscular dystrophy, Neurodegenerative disease, Neuropathies, Opioid Use Disorder (adjuvant therapy), Human Immunodeficiency Virus (HIV) and Acquired Deficiency Syndrome (AIDS), Post-Traumatic Stress Disorder (PTSD), Seizure disorder, Sickle cell disease, terminal illness with prognosis of less than 12 months to live, Tourette Syndrome are eligible for medicinal cannabis in PA.

REGISTER ON PA DEPARTMENT OF HEALTH'S WEBSITE

Patients will have to provide their name, demographics, email, and PA driver's license or PENNDOT state issued card number. Caregivers can register for a child, a patient with a disability, or a patient that needs in-home support (in the case you will need the caregiver's driver's license or state ID, not the patient's.)

BOOK AN APPOINTMENT ONLINE

New patients will book online. You will also complete online forms and a provider will review your medical history. Instructions for payment will be emailed to you once you book online. Make sure you monitor your email inbox and spam, just in case.

COMPLETION!

Your online paperwork is completed. Our fee is paid. Your education and provider appointment is complete. The provider will go over any drug interactions or concerns during your appointment. We recommend that moderate and high risk patients have a follow up visit, but is up to the certifying provider. The provider will coordinate with Department of Health and confirm with you within 24 hours. At that point you would go to the Department of Health's website and login ( the username and password you created when you register). The fee for the patient or caregiver medical marijuana ID card is $50. Medicaid, PACE/ PACENET, CHIP, SNAP and WIC may be eligible for no-cost cards. Allow 7 days for the state to print the card and an additional 14 days in the mail.
